New Patient Information
Your First Visit
When you come to Knutzen Family Dentistry for the first time, we’ll do everything we can to make you feel like you’ve been with us for years. You’ll be greeted by a friendly member of our team at the door, and they’ll help you quickly organize your paperwork and will answer any questions you might have.
You’ll then be escorted to a treatment room where you’ll meet one of our hygienists, and they’ll take some x-rays and perform a dental cleaning. After this, you’ll get to meet Dr. Ben Knutzen or Dr. Stacey Borowski. They’ll take a moment to get to know you and give you a chance to talk about some of your dental concerns and goals.
From there, they’ll perform a thorough oral exam. With this information, they’ll then come up with a treatment plan that will meet all of your individual needs. You’ll get a chance to ask all the questions you like, and then you’ll be escorted to the front desk where we’ll help you schedule your next appointment.

Want to save 15 minutes at your next appointment? We thought so! All you need to do is fill out our patient forms beforehand, which you can do right here. Simply print them, fill them out, and bring them to your next visit to help things fly by even faster. If you have any issues getting this to work, please let us know! You can also choose to fill out the forms at our office if that is more convenient.
